Aging at Home is a preference for many. Home is where the heart is! If you are searching for a private PSW or Home Support person to meet your needs and retain your goal to stay at home, the screening and selection process can be overwhelming. Lifetime Transitions can assist with recruitment and screening candidates to meet your tailored needs. Pre-screening and interviewing of candidates is offered to reach a final selection of candidates for you to review, meet and participate in your final decision of a candidate that best suits you.

Are you in the process of entering Long Term Care? Prior to selecting homes to potentially live in, you wshould set up appointments to visit Long Term Care Homes in your area. It is the tours where you can pick up a “vibe” of the home. Lifetime Transitions can create a customized checklist based on your felt needs, preferences and things that are important to you in your living home environment. A 30 minute to one hour meet up with you is needed to discuss your perspective of Quality of Life while living in Long Term Care. A checklist  is then created with a scoring scale to take with you on each of your visits. The checklist reflects standarfized questions which reflect compliance with the FLTCH Act integrated with your important points of interest. 

Advocacy Support & Knowledge

With over 20 years experience working in LTC in various positions as Administrator, Recreation & Support Services Supervisor &Environmental Services, Lifetime Transitions offers the ASK Program which simply is a support program to assist with questions, conflict resolutions and solutions to different situations you may feel you need support for. Advocating for Resident centred care and Resident Rights are at the forefront of our support.

This program is offered at an hourly rate, however your time can be taken in  minutes that may build up over a period of time.

For example: You need some support dealing with a process you are not familiar with, this may require a 6 minute phone call. Therefore, you still have 54 mins left in your support time to utilize at a later date.
